LMRA Checklist Template
A Last Minute Risk Assessment checklist helps teams verify the actual task situation before work begins. It is short by design: the goal is to confirm that hazards, people, tools, and controls still match the plan right at the point of execution.
Checklist items
- Task scope and location.
- Current hazards and nearby activities.
- Required permits, isolations, and PPE.
- Team readiness and communication.
- Stop, proceed, or escalate decision.
Suggested checklist prompts
- Has the task changed since it was planned?
- Are all energy sources controlled and verified?
- Are people clear on their roles and the stop-work signal?
- Is the area safe, accessible, and free of unexpected interference?
- Have we agreed on what to do if conditions change during the task?
How to use this template
Use the checklist at the work area, not in a meeting room. If the answer to any prompt is unclear or unsafe, stop the task until the condition is corrected or escalated.
